This section contains information about services that could be available to you to meet your social care needs and advice about other aspects of social care support for children and young people aged 0 – 25 who have a disability or additional need.

A small number of people need this kind of help. Despite the name, learners with Education Health & Care Plans do not often need ongoing social care support.

The services we can provide to families of children and young people with disabilities assessed as a ‘child in need’ include:

  • short breaks – during the day or overnight
  • support to enable a disabled child to access community-based leisure activities
  • support in the home with the care of a disabled child
  • support to access adaptations and special equipment

The Children with Disabilities Team (CWDT) works closely with other services for disabled children to ensure families experience a joined up service.
